These terms set out the conditions for online dietetic and psychodietetic services provided by Monika Chojnacka, conducting business as 23 Monika Chojnacka, ul. Niemczańska 33/6, 50-561 Wrocław, Poland, Tax Identification Number (NIP): 7692204980.
Definitions
Patient: any adult using the Dietitian’s services and any minor using them with the written consent of a legal guardian.
Dietitian: the service provider Monika Chojnacka, conducting business as 23 Monika Chojnacka, ul. Niemczańska 33/6, 50-561 Wrocław, Poland, Tax Identification Number (NIP): 7692204980.
General provisions
Registration for an online visit is considered acceptance of the conditions set out in these terms.
These terms apply to services offered online. Due to their nature, the conditions for other services are agreed individually.
Registration for visits and all communication take place by phone at 533 615 575 and by email at [email protected].
Visits take place online using Google Meet.
Visits do not replace a medical appointment and are informational in nature.
Cancelling and rescheduling visits
A visit may be rescheduled no more than three times. After that, the service is considered provided.
Cancellation of a paid visit by the day before it is due to begin may result in a refund of the costs incurred at the Patient’s request.
Cancellation of a paid visit on the day it is due to begin may result in no refund of the costs incurred.
A paid visit that was not cancelled may be booked again no later than 30 days after its original date. After that, the service is considered provided.
The Dietitian may refuse to provide a service to the Patient, and this does not constitute a breach of contract, when the Patient: is under the influence of alcohol, intoxicants or psychoactive substances; requires medical or other intervention beyond the Dietitian’s competence; refuses to provide necessary information about their health, lifestyle or eating history; refuses to complete necessary diagnostics; behaves aggressively or offensively, or infringes the personal rights of the Dietitian or third parties; breaches these terms; or is in a situation that threatens health or life.
Scope of services
We plan the scope of services and frequency of visits together.
Services provided: online dietetic and psychodietetic visits, nutrition plans, educational talks and sessions for children and adults, and plant-based cooking workshops.
Prices
- Online dietetic/ psychodietetic visit, 90 minutes: PLN 250.
- Support visit for members of the patient’s family, 90 minutes: PLN 250.
- Individual nutrition plan: PLN 400.
- Educational talks and cooking workshops: priced individually.
Payment method
Traditional bank transfer before the visit begins, using the details provided by email.
Patient
Declares that the information provided is true.
Understands that the effectiveness of dietetic and psychodietetic actions depends on many factors, not only the Dietitian’s services, and that services provided professionally do not guarantee the outcome expected by the Patient.
Is required to provide a stable internet connection and a functioning device that enables participation in the online visit.
Declares explicit consent to the processing of special categories of personal data, including health data, in accordance with Article 9(2)(a) of the GDPR.
Dietitian
Declares that all information provided by the Patient will be kept professionally confidential.
Performs her duties with care and diligence for the Patient’s wellbeing.
Is not liable for consequences resulting from failure to follow recommendations.
Final provisions
These terms may change, and the Patient will be informed electronically.
A statement submitted within 14 days after notification of changes to the Terms, indicating that the Patient does not accept the new Terms, is equivalent to ending the provision of services.
Matters not covered by these Terms are governed by Polish law.
These Terms in their present form take effect on 1 January 2026.
